Delhi has built one of India’s most advanced healthcare ecosystems, serving over two crore residents and acting as a leading referral hub for tertiary and quaternary care across North India. However, meeting healthcare demand now requires more than expanding infrastructure. Rising non-communicable diseases (NCDs), fragmented care pathways, affordability challenges and uneven use of existing capacity are putting greater pressure on the healthcare system. These challenges highlight the need for a more integrated, patient-centric approach to care.
This report, developed by Grant Thornton Bharat, in association with CII Delhi, presents a healthcare transformation roadmap for building a connected, digitally enabled and sustainable healthcare ecosystem. The report outlines how collaboration among policymakers, healthcare providers, industry, academia, startups and technology innovators can improve healthcare delivery, strengthen resilience and support better health outcomes.
Delhi’s healthcare ecosystem today includes approximately 3,878 healthcare institutions and nearly 63,800 hospital beds, with the private sector contributing more than 53% of total bed capacity. The city also houses more than 180 NABH-accredited healthcare facilities and is witnessing growing adoption of digital health, telemedicine and AI-enabled healthcare solutions.
While Delhi has expanded its healthcare capacity, several structural challenges continue to affect system performance. Tertiary hospitals often face overcrowding, while private healthcare facilities remain underutilised. Many patients still struggle with affordability and fragmented care pathways can disrupt continuity of care. At the same time, the rising burden of chronic diseases such as diabetes, cardiovascular diseases, cancer, respiratory illnesses and mental health conditions is increasing the need for more coordinated and long-term care models.
The first pillar focuses on building a stronger continuity of care through integrated service delivery and coordinated patient journeys. It aims to improve how patients move across primary, secondary and tertiary care settings while ensuring continuity in treatment, follow-up and long-term care.
Key priorities in this framework include coordinated referral pathways across levels of care, public-private collaboration for diagnostics and specialised services, integrated chronic disease management, mental health integration within care delivery models and improved patient experience, quality and safety standards.
The report highlights the importance of shifting from episodic treatment to coordinated, longitudinal care that supports patients throughout the healthcare journey.
Digital transformation is emerging as one of the most important drivers of healthcare reform. As healthcare systems become increasingly connected, digital technologies can improve accessibility, efficiency and clinical outcomes while supporting more effective coordination of care.
The report identifies several priorities for digital healthcare transformation, including Ayushman Bharat Digital Mission (ABDM) and Ayushman Bharat Health Account (ABHA)-enabled digital health infrastructure, interoperable electronic health records, telemedicine and hybrid care models, remote patient monitoring, point-of-care diagnostics, home healthcare and hospital-at-home services and digital care coordination platforms.
A digitally connected healthcare ecosystem can support seamless information exchange, improve referral efficiency, reduce duplication of diagnostics and enable more coordinated decision-making across providers.
Long-term healthcare transformation requires more than clinical excellence and digital innovation. It also requires sustainable financing, resilient infrastructure and governance frameworks that support improved outcomes and system-wide efficiency.
The Thrive pillar focuses on outcome-oriented public-private partnerships (PPPs), value-based healthcare models, innovative financing, workforce development, climate-resilient healthcare infrastructure and data-driven governance to build a sustainable and resilient healthcare system.
By aligning healthcare delivery with long-term sustainability goals, Delhi can strengthen affordability, improve resource utilisation and enhance system resilience.
AI in healthcare is moving from experimentation to implementation. As healthcare providers seek to improve accuracy, efficiency and access, artificial intelligence is emerging as a critical enabler of future-ready care models.
The report highlights the role of AI across multiple areas of healthcare delivery, including AI-enabled diagnostics, clinical decision support, predictive analytics and risk stratification, disease surveillance, population health management, remote patient monitoring and operational planning and resource optimisation.

Achieving this vision will require stronger care coordination, universal digital integration, technology-enabled service delivery and greater collaboration across the healthcare value chain.
